Lip filler is a non-surgical treatment in which a hyaluronic acid filler is injected into the lip tissue to add volume, shape or border definition. The effect is temporary and can be reversed with an enzyme if necessary.

This page explains what lip filler is, what it is used for, how the treatment is carried out, how long it lasts and what the risks are.

What is lip filler?

The aim of lip filler is not simply to add volume; balancing the lip border (vermilion line), the philtral columns and the ratio of upper to lower lip is part of the plan. A published framework study recommends assessing the integrity, proportion and structure of the lips together to achieve a natural result.

Hyaluronic acid is a water-binding substance that occurs naturally in tissue. Soft, flexible formulations are preferred for the lips.

Who is a candidate?

Where the distance between the upper lip and the nose is long, filler alone may not be sufficient; in that case a lip lift can be planned together with it.

How is the treatment performed?

The treatment is carried out in the practice and takes 15–30 minutes.

  1. Assessment: lip proportions, borders and asymmetry are examined; the amount and placement plan are discussed with the patient.
  2. Preparation: the area is cleaned; numbing cream is applied.
  3. Injection: the filler is placed in small volumes in the appropriate layer of the lip with a needle or blunt cannula, with attention to the vascular anatomy.
  4. Shaping: smoothed with gentle massage; symmetry is checked.
  5. Afterwards: hot drinks, make-up and pressure on the lips are avoided for the first 24 hours.

How does the effect develop?

What are the risks?

The list below is not exhaustive; individual risks are discussed at the consultation and during the informed-consent process.

  • Swelling, bruising, tenderness
  • Asymmetry, irregularity, filler that can be felt
  • Reactivation of cold sores (herpes)
  • Infection, late nodules
  • Migration of the filler
  • Vascular occlusion: important vessels run around the lips. Filler entering a vessel can compromise the blood supply of the skin; it is rare but serious. Published reviews stress slow injection, small volumes and early hyaluronidase treatment.
  • Vision-threatening vascular occlusion: a very rare event reported with facial fillers that requires emergency treatment.

How is the decision made?

At the consultation the lip proportions, the definition of the borders, asymmetry, the distance between nose and upper lip and the lines around the mouth are assessed. Whether the goal is volume, shape or border definition is clarified.

Keeping the amount measured and, if necessary, completing the treatment in a second session helps to preserve a natural look. Because the filler is temporary, the result can be evaluated and dissolved if necessary.

Frequently Asked Questions

Will my lips look overdone?

The appearance depends on the amount used and how it is placed. A measured treatment that respects the lip's own proportions and borders aims to keep a natural look. The amount is agreed together at the consultation.

Which filler is used?

Soft, flexible hyaluronic acid fillers are preferred for the lips. Hyaluronic acid occurs naturally in the body and can be dissolved with an enzyme if needed.

How long does it last?

Because the lips move constantly, filler is absorbed faster there than elsewhere; the effect usually lasts 6–12 months, depending on the person and the product.

Does it hurt?

The lips are sensitive; with numbing cream and the local anaesthetic contained in the filler, most patients tolerate the treatment with only a slight sting and pressure.

How long does the swelling last?

Lip swelling is most noticeable in the first 2–3 days and largely settles within a week. The final appearance becomes clear in 1–2 weeks.

I get cold sores — can I have lip filler?

In people who get frequent cold sores, injection can trigger an outbreak. Preventive antiviral treatment can be planned before the procedure; it is not performed during an active cold sore.

What if I am not happy with the result?

Hyaluronic acid filler can be partly or completely dissolved with the enzyme hyaluronidase. This is one of the reasons permanent, non-hyaluronic fillers are avoided in the lips.

Can it be combined with a lip lift?

Where the upper lip is long, filler alone may not be enough; filler can be planned together with, or after, a lip lift operation. The decision is made at the consultation.
